Things to remember :
You must carry your original Aadhar Card / Passport / PAN Card / Voter ID / Driving License.
You must carry your Previous Vaccination Certificate in case of 2nd dose.
You must carry your Prescription in case of previous or current morbidity.
During visit to the hospital, we will ask you to authenticate your booking as below :
Show the screenshot of your Booking ID at the counter.
Show your valid original photo ID (any one of the mentioned documents : Aadhar Card / Passport / PAN Card / Voter ID / Driving License).
There might be a delay in services due to several conditions / reasons including emergencies
SRI SAI HOSPITAL reserves the right to cancel / modify booking any time without intimation. You are advised to re-check status of your booking before hospital visit for your convenience.
